Initial Consultation
If you have questions/concerns about working with me, I will be happy to offer a free phone consultation or you can email me with general inquiries.

Confidentiality
Confidentiality is essential in the treatment process. Your rights and limits to confidentiality are discussed during the first meeting.

Insurance
I am currently an In-Network provider for United Behavioral Health/Optum and Carelon/Beacon Health. If using Out-of-Network benefits, I will be able to provide you with payment statements. These statements may qualify you to then privately obtain out-of network reimbursement through your insurance company. It is recommended that you verify out-of-network behavioral health/mental health benefits with your insurance company prior to the first visit.

Suggestions of Questions to Ask Insurance
*What is my out of network deductible? *What is the coverage period of my deductible? *What percentage of the billed amount will you pay for each session? *What is the maximum number of sessions per calendar year? *How do I get reimbursed? *How long does it take to get reimbursed?

Counseling Fees
When using In-Network benefits, Assessments, Individual and Couples sessions are charged at the contracted In-Network insurance rate. If choosing to pay privately, sessions are charged at a rate of $125 / 45 min sessions.
I will be glad to review any fees and health insurance information during your phone consultation with me.

Accepted forms of payment include:  MasterCard, Visa, American Express and Discover card. Clients are responsible for payment (including any applicable co-pays) at the time services are rendered.  A credit card on file is required and will be used to bill for payment of rendered services.

Cancellation Policy
If you need to cancel an appointment, 24 hours notice is required to avoid being held responsible for the full fee. Any emergency situations will be discussed on a case-by-case basis.
